1. When do I become a Participant in this Plan?
You become a Participant on the date your Employer is required to make its first contribution to the Fund on your behalf.
2. When are my Benefits Vested?

Once you have earned 5 years of Vesting Service you attain a non-forfeitable right to Benefits under the Plan.

3. When am I eligible to begin receiving my Benefits?

There are a number of circumstances under which you are eligible to begin receiving benefits:

Normal Retirement
Disability Retirement Pension
Early Retirement Pension

4. In what form my Benefits be distributed?

You may receive your benefits in one of the following manners:

50% Qualified Joint & Survivor – You will receive reduced monthly payments during your lifetime, so that after your death your surviving spouse will receive a payment equal to 50% of your monthly benefit.
75% Joint & Survivor – You will receive reduced monthly payments during your lifetime,so that after your death your surviving spouse will receive a payment equal to 75% of your monthly benefit.
Single Life – A monthly payment for your lifetime. If you are married, your spouse must consent to this form of payment.

5. How do I find out about my eligibility and beginning my benefit?

Statements showing your estimated monthly benefit are mailed annually. If you have any questions or wish to obtain an application you should contact the Fund's Administrator.
